Making a referral and scheduling an appointment is easy.
Appointments to BioMotion Labs generally require a referral from your doctor. Please have your physician complete the appropriate referral form and submit it to us for insurance authorization, this will speed the process of scheduling appointments and help ensure payment by your insurance company.
Please note some services are not covered by insurance and are available on a private pay basis.

If you are seeking an appointment as a result of a work-related injury, auto accident or other disability please use our Adult referral form. Job Description & Physical Demand requirements are an important part of the evaluation process. We have provided an easy to use form to be completed by you or your employer.
Preparing for your motion lab appointment
Your motion analysis appointment can take up to 2-3 hours depending on the specific request from your physician. On the day of your appointment please do not use any lotions or creams to your skin as it can slow down the evaluation process.
What to Bring to Your Appointment
We recommend that you bring or wear comfortable clothing such as athletic shorts, tank top and/or a bathing suit. Ideally, the clothing will allow you to move freely so that we can easily collect data. It is important that we are able to see your lower and upper body in motion and while performing functional tasks as requested by your physician.
Please bring any assistive devices that you may use, such as braces, crutches or walkers. We will watch and evaluate your movement with and without these devices.
Before your appointment, patients should complete any forms you have been asked to complete and bring it with you to your appointment. This information is necessary before we are able to begin your evaluation.
